Our staff looks forward to meeting you and discussing your goals. We strive to keep our appointments on time as scheduled. Prior to your appointment, we encourage you to fill out the patient forms that are located on this website so you can accelerate your check-in process. Please download, print, and complete the forms and bring them with you to your appointment. If you are unable to download our patient forms, then it is recommended that you arrive at your appointment 15 – 30 minutes early so you can have ample time to fill out the appropriate forms.

Insurance and Policies

We accept most major health insurance plans including Blue Cross Blue Shield, Cigna, Aetna, United and Humana.

Please call our office if you would like to inquire specifically to see if we are contracted with your health insurance plan. If certain services are not covered under your plan, you will be responsible for all non-covered service charges. If we do not have a contract with your plan, we can provide you with medical care out of network as fee for service but require payment at the time of your visit.

Payment Options

Payment is expected at the time medical and cosmetic services are rendered. For your convenience, we accept many insurance plans and offer an array of payment options, such as cash, check, and most major credit cards including: MasterCard, VISA, American Express and Discover. Additionally, we also offer online payment available through our website to pay any outstanding invoice balances more easily.
