Alopecia areata is caused when the immune system erroneously attacks the hair follicles, leading to rapid loss of hair. Androgenetic alopecia results from inheriting hair follicles that react negatively with dihydrotestosterone (DHT), a derivative of the testosterone hormone, resulting in damaged or scarred follicles. In both situations, the hair regrowth cycle is stopped and hair loss occurs. Telogen Effluvium is a self limited loss of hair, or overall hair thinning, that occurs after a significant life event such as after having a baby, loss of a loved one or being hospitalized for an illness. Further, some hair loss can occur from vitamin, mineral or hormone deficiencies and blood work may detect these issues and treatment can be directed toward resolving the specific deficiencies.
The major symptom of alopecia is hair loss. Hair loss usually is seen on the scalp, but might also occur on other parts of the face and body. You may first notice clumps of hair in your hairbrush or in the drain of the shower. Your scalp may be itchy or have other symptoms that should be discussed with Dr. Kingston. On examination, Dr. Kingston may see specific findings or clues that suggest one etiology over another.
Depending on the cause of hair loss, different treatment options are available. For example, Dr. Kingston may suggest certain oral supplements if deficiencies are noted, or she may prescribe oral medications such as spironolactone and finasteride. She might suggest BioTe hormone replacement and optimization if hair loss is related to hormone imbalance. Topicals such as Rogaine or steroid creams may be used. Additionally, Intralesional steroid injections or even oral steroids might be a part of your personalized treatment plan. At times, PRP therapy, which uses your extracted growth factors to encourage the regrowth of damaged hair follicles, or even hair transplant surgery may be recommended.
